As a patient of our practice, you agree to:

• Provide accurate and complete medical history and health information
• Follow treatment plans and medical advice as prescribed
• Attend scheduled appointments or provide adequate notice for cancellations
• Pay for services rendered according to our payment policies
• Treat our staff and other patients with respect and courtesy
• Comply with office policies and procedures
• Notify us of changes to contact information or insurance coverage

Cancellation Policy: We require at least 24 hours notice for appointment cancellations. Repeated no-shows or late cancellations may result in discharge from the practice.

Payment is due at the time of service. We accept cash, checks, and major credit cards. For patients with insurance:

• You are responsible for verifying your insurance coverage and benefits
• Co-payments, deductibles, and co-insurance are due at time of service
• You are responsible for any services not covered by your insurance
• We will file claims on your behalf as a courtesy, but you remain ultimately responsible for payment

Unpaid balances may be sent to collections and may affect your ability to schedule future appointments.

To the fullest extent permitted by law, NJ Primary and Sports Medicine, Dr. Joseph Riggi, and our staff shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from:

• Your use or inability to use our website
• Errors or omissions in website content
• Unauthorized access to our systems
• Technical failures or interruptions

Medical Disclaimer: Nothing in these Terms limits our liability for medical malpractice or gross negligence in the provision of medical services.
